Once provided an assignment, I agree to show up on time and stay the length of the assignment. I understand the price will be negotiated prior to the original contract being offered to me and once I accept that amount, that is the amount of length of stay with that client. Any discussion with the client or the Clients representative or Family about how much they have paid for services is fully prohibited. I understand that I will not solicit the client to pay me in any way, shape or form, or try to get the client to hire me directly for services. Any of these things can be immediate dismissal and forfeiture of pay.

I understand and agree that I represent the Addictions Academy and Dr. Cali Estes' brand, and will act in a manner as such. I will dress appropriately, act appropriately with all clients at all times, and not engage in any illicit or illegal activities, (including drinking or drugs) at any point, on my own or with clients.

I understand and agree that if something goes wrong and I need to be removed, I will call Dr Cali Estes immediately and inform her of what is going on so that I could be replaced in a quick and timely manner. I understand that I will not just walk off the job And abandon the client.

I understand and agree that the base pay includes my daily rate, and any additional expenses the client will pay. This includes daily food, any travel to and from the client, or during my stay with the client, as it benefits to the client. I understand if the client takes me out to dinner they will pay for that and any extraneous activities will be covered.

I understand and agree that I will be providing services for that which I was hired and not outside the scope of my profession.

I understand that once an assignment is over with a client I will not contact the client in any way: phone, text, email, or mail. I understand that all clients are Property of The Addictions Academy, DBA Sober on Demand.

I understand and agree that if I have any questions, I will immediately call Dr. Cali Estes and her team with any questions or concerns that I have.

I understand and agree that I will be keeping daily note logs of what’s going on with the client and checking in with Dr Cali Estes and her team daily.

I understand an email will be provided to me with the clients information and the pay rate whether it’s hourly or daily in that email, and once I replied to that with an agreement, it will be considered a binding contract for that rate.

If you are in the role of a Sober Companion or Nurse and you need to be replaced, you need to remain with the client until a replacement staff member can take your position. Walking off the job will be considered client abandonment and forfeiture of any pay to that point.

I understand and agree that if I failed to comply with any of these terms and conditions, my pay rate will be subjected to the lowest pay rate that is currently offered, and I can be immediately terminated.
